An advanced health care directive (“AHCD”) names someone to make health care decisions for you if you cannot. This will also let your family and physicians know of your wishes for special treatment that you may want, or do not want, near the end of your life. An AHCD allows you to make specific statements of your desires regarding your health care should you become incapacitated. An AHCD may also give an agent the authority to withhold consent to, or to direct the withdrawal of, any form of medical treatment, including life-sustaining treatment. An AHCD also allows an individual to direct or limit organ donation.
